Our primary care providers Hector Delgado, DO, Yusely Hernandez, ARNP and Vanessa Delgado, PA, specializes in preventive care and annual wellness check ups, and Geriatric specialize, acute or chronic conditions and follow up visits. We strive to give the best care possible to all of our patients. We provide same day appointments, Walk-in appointments, Referrals Specialist in 24 hrs.
Laboratory
We provide convenient on-site laboratory services to all of our patients.
Diabetic Education
We help support and educate diabetic patients with personalized careplans. In order to effectively monitor a diabetic patients glucose and A1C levels, routine labs are scheduled every 3 to 6 monts.
A personalized care plan includes; treatment goals, glucose monitoring, medication and insulin therapy information, nutrional advice, physical activity plans, mental health considerations smoking cessation advice and obesity management.
Telemedicine
Primary care at a distance!
We offer telemedicine services for Geriatrics patients that are physically unable to come to the office via Televideo chats and audio calls with one of our experienced providers.
PRE-OPERATIVE CLEARENCE
We provide a complete evaluation including diagnostic test, laboratory test and in some cases specialists evaluation prior to an elective surgery.
